 At AYWA CARE SUPPORT SOLUTIONS, we proudly provide care for individuals with a wide range of disabilities and complex health needs. Our experienced team of RNs, EENs, and Support Workers is trained to support clients living with:

Our Nursing Team Provides:

  • Medication Management
    Safe administration, monitoring, and review of medications, including complex medication schedules.

  • Wound Care
    Treatment, monitoring, and ongoing management of chronic or acute wounds, pressure injuries, and post-surgical care.

  • Vital Signs Monitoring
    Regular checks of blood pressure, pulse, temperature, oxygen levels, and blood sugar to ensure ongoing stability.

  • Chronic Disease Management
    Support for conditions such as diabetes, MS, MND, Parkinson’s Disease, cardiovascular issues, respiratory conditions, and more.

  • Catheter & Stoma Care
    Management of urinary catheters, stoma bags, and bowel care support.

  • PEG/Tube Feeding Support
    Assistance with enteral feeding, nutrition monitoring, and tube site care.

  • Pain Management
    Assessment and strategies to reduce discomfort and promote quality of life.

  • Post-Hospital & Recovery Care
    Nursing support after surgery, injury, illness, or hospital discharge to ensure safe recovery at home.

  • Clinical Assessments
    Health assessments, risk assessments, and ongoing care reviews to ensure clients receive the appropriate level of support.

  • Behavioural & Mental Health Support
    Compassionate assistance for clients with psychosocial disabilities, behavioural concerns, or mental health-related needs (within nursing scope).

  • Education & Training for Families
    Teaching carers and families safe techniques for daily care, equipment use, and condition management.

Our Support Workers Provide:

  • Personal Care Assistance
    Support with showering, dressing, grooming, toileting, hygiene, and daily routines.

  • Domestic & Household Support
    Cleaning, laundry, meal preparation, shopping, and helping maintain a safe, organised home.

  • Community Access & Transport
    Escorting clients to appointments, social activities, shopping, community events, and recreational outings.

  • Social & Emotional Support
    Companionship, conversation, engagement in hobbies, and reducing social isolation.

  • Support with Mobility & Transfers
    Safe assistance with walking, transfers, hoist use, and mobility aids to promote independence and safety.

  • Medication
    Non-clinical reminders to take medication on time (within support worker scope).

  • Skill Building & Capacity Building
    Assisting clients to develop life skills such as cooking, budgeting, communication, planning, and personal organisation.

  • Respite for Families or Carers
    Providing short-term support so families and carers can take a break with peace of mind.

  • Behavioural Support (within scope)
    Supporting clients with routines, structure, and behaviour strategies under guidance from clinical or specialist teams.

  • Support for Complex Needs (with RN/EEN oversight)
    Working alongside nurses to provide consistent daily support for clients with physical, intellectual, or neurological disabilities.
